Allegations that staff administered cocaine or PCP to a resident and that a resident sustained an unexplained head injury were likewise disproved; medication records and clinical reviews found no evidence of illicit drug distribution, and documented fall protocols appeared to have been followed. Claims of medication mismanagement and unauthorized billing were examined through prescription logs and pharmacy records, revealing that all medications were dispensed per physician orders and that billing responsibilities were correctly assigned to residents or their representatives.

All four investigations reaffirmed the facility’s compliance with state licensing requirements. No violations were identified, and the investigators noted that residents generally perceived staff as kind, respectful, and attentive. The reports remain public for three years, ensuring transparency for families and regulators. While the allegations were dismissed, the repeated focus on medication handling, resident safety, and safeguarding practices underscores the importance of continuous monitoring and proactive communication with residents and families to maintain trust and uphold the facility’s standards.
