Woodhaven Village Senior Living underwent a comprehensive inspection on July 13, 2021, resulting in 10 state standard violations. The majority of these deficiencies were related to Life Safety Code requirements, including fire safety and building maintenance issues. Specifically, the facility was cited for failing to ensure compliance with Chapter 18 of the Life Safety Code, NFPA 101, as well as inadequate fire drills, emergency plans, and fire alarm documentation. Additionally, there were concerns regarding interior walls and ceilings, sprinkler systems, and portable fire extinguishers. Health Code deficiencies were not listed during this inspection. The facility had previously corrected similar issues between April 16, 2019, and May 15, 2019. On February 10, 2025, the facility received an administrative penalty of $2,450 for failing to have an ongoing and effective pest control program and providing insect screens on operable windows.
