The LPA also noted that fire‑extinguishers and the sprinkler system were in service and that disaster drills had been conducted quarterly.

Across the three visits, a pattern emerges: initial compliance with basic hygiene and licensing requirements gave way to emerging safety risks related to resident self‑care environments and regulatory documentation.
