While the facility had knowledge of the vaccinations, it could not confirm dates at the time of the provider’s request, and no regulatory violation was identified.
Across the three reports, a recurring theme is gaps in routine care processes—cleaning and medication administration—often linked to staffing limitations or insufficient oversight. The July and October findings led to actionable corrective measures, whereas the November case highlighted an information gap that did not meet the threshold for a regulatory deficiency.
