The citations were issued during three separate on‑site inspections. The facility’s total number of citations is 18, which is below the state average of 31 for nursing homes.

The inspection that generated the largest number of cited items occurred on March 17 2022 and was a combined Certification/Complaint Survey. This inspection identified nine deficiencies, comprising two Standard Health citations—“Activities meet interest/needs each resident” and “Nutrition/hydration status maintenance”—and seven Life Safety Code citations, including “Building construction type and height,” “Illumination of means of egress,” “Means of egress – general,” “Smoke detection,” “Sprinkler system – installation,” “Stairways and smokeproof enclosures,” and “Vertical openings – enclosure.” These deficiencies highlight concerns in both resident care activities and the facility’s fire protection and egress systems.

Enforcement actions taken against the facility include two stipulations and orders. The first, Stipulation & Order # NH‑22‑177, was issued on December 20 2022 following a survey on May 17 2020 and imposed a fine of $8,000 for Quality of Care deficiencies.
