Inspection Report Summary for Upper East Side Rehabilitation and Nursing Center

During the reporting period, comprising 8 standard health citations and 5 life safety code citations. These citations were issued in two on‑site inspections that resulted in citations, while 12 additional inspections were conducted and found to have no citations. No citations involved actual harm or immediate jeopardy to residents.

The inspection conducted on January 23, 2024 generated the largest number of cited items, with nine citations in total. Four standard health deficiencies were identified, including issues with food procurement, pest control, menu preparation, and resident participation in care planning. Five life safety deficiencies were also cited, covering egress door functionality, electrical equipment and cord safety, gas equipment storage, illumination of means of egress, and maintenance, inspection, and testing of doors. The August 30, 2021 certification/complaint survey produced four standard health citations related to resident mobility, pharmacy services, medically related social services, and reporting of alleged violations.

Complaint data for the period from August 1, 2021 to July 31, 2025 show that the facility received 221 complaints, equating to 47.4 complaints per 100 occupied beds. Sixty‑two percent of these complaints represented facility‑reported incidents. The Department of Health conducted 18 complaint‑related on‑site inspections, resulting in three citations and a rate of 0.6 citations per 100 occupied beds. Complaints‑related citations by category totaled 2, with one citation each for resident rights and other services; the category totals for administration, dietary services, physical environment, and quality of care were zero. The total number of complaints‑related citations reported as 3 differs from the category subtotal of 2, and this discrepancy is noted.
