The medication error likely contributed to the resident’s disorientation and elopement from the unlocked facility. A June 20, 2025, license renewal authorized continued operation with a maximum capacity of 40 beds. The facility’s occupancy has declined from 46 residents in June 2019 to 31 residents in June 2025.
