A 25-bed community in Deforest, Wisconsin, The Legacy of DeForest covers three levels of care under one address: independent living, assisted living, and memory care. Seasonal menus rotate through the dining room, giving some variety across the year.

Pricing runs from $7,910 to $8,910 a month, depending on care level and accommodation type, a range wide enough that it’s worth a direct conversation about where a specific resident would land. Wisconsin’s Department of Health Services, Division of Quality Assurance, oversees inspections statewide, and here those have tended to focus on staffing credentials and how well care plans get documented.

What stands out is the structure itself: three care levels in one small community, built for a resident who might start more independent and need more support later, without ever having to pack up and move somewhere new.
