Inspection History
Inspection Report Summary for Westwind Memory Care
Westwind Memory Care Facility (Facility Number 445202597) has faced a series of regulatory interactions over the past three years. In March 2021 a non‑compliance conference was convened after a complaint was substantiated on February 1. The licensing team outlined cited deficiencies, and the administrator, wellness director, and vice president indicated that corrective actions—including training and revised reporting policies—had been implemented. A compliance plan was drafted and approved, and no further deficiencies were recorded during the 3/8/21 meeting. Subsequent inspections in July 2021, August 2022, and April 2023 focused on verifying adherence to that plan and observed no new violations; staff interviews confirmed that fall‑assessment and reporting protocols were being followed, and the facility’s environment remained clean and well‑maintained.
In December 2021, an unannounced annual inspection highlighted robust COVID‑19 mitigation practices: staff and residents were vaccinated, a booster clinic had been held, and the facility maintained adequate supplies of PPE and food. All safety equipment, including fire extinguishers and emergency exits, was inspected and found compliant. These findings reinforced that Westwind was maintaining regulatory standards in health and safety during the pandemic.
However, a 4/19/2023 case‑management visit revisited allegations from an earlier investigation that had culminated in a resident’s death in August 2020. The Department concluded that the facility failed to observe and assess a bruise and fracture after an un‑witnessed fall, did not promptly report the incident to hospice or the Department, and violated several sections of the California Code of Regulations (CCRs) concerning personnel, observation, and reporting requirements. The investigation documented gaps in incident reporting and communication with hospice providers that likely contributed to delayed medical care and the resident’s eventual demise. This case underscores a persistent deficiency in incident documentation and inter‑agency communication that contrasts with the otherwise compliant environment reported in later inspections.
Overall, Westwind Memory Care has demonstrated a capacity to respond to and correct specific regulatory deficiencies, maintaining a safe and clean facility environment in most subsequent visits. Nonetheless, the 2023 findings reveal a serious lapse in incident reporting and coordination with hospice care, raising concerns about consistent adherence to reporting protocols and the potential impact on resident safety.
