Oakmont of Carmichael, a 740‑type facility with a census of 71 residents, was the subject of a California Department of Social Services unannounced inspection on 11 October 2022. Evaluator Sabrina Calzada reviewed documentation and conducted interviews with the administrator Kathleen Gilbey, past and present memory‑care directors, staff, and the resident (R1) who had moved into the memory unit from a skilled nursing facility in August 2021. The investigation centered on complaints filed on 19 May 2022 alleging inadequate hygiene support, a missing shower chair, a rash, and unsatisfactory wound care.
The evaluator found the claims that R1 had been denied a shower chair for an extended period and that the resident developed a rash while in care to be substantiated. Staff records show a shower chair was initially provided but then misplaced or shared with a roommate, leaving R1 to shower in a wheelchair until a new chair was finally issued in September 2022. Multiple staff interviews and physician fax reports documented episodes of guided falls, resistance to bathing, and weeks without showering, which contributed to recurrent skin breakdown. While the facility did follow physician orders for wound‑care products, gaps in medication delivery and inconsistent application of creams were noted, but these deficiencies were not sufficient to substantiate the allegation that wounds were improperly treated.
The complaint that staff falsely documented that R1 had received medical attention for the rash was likewise unsubstantiated. Although faxed physician reports and emergency‑department visits were documented, the investigation could not determine that a Med‑Tech had intentionally misrepresented care. The findings highlight a pattern of delayed equipment provision and inconsistent hygiene practices, underscoring the need for improved protocols and oversight to ensure timely, appropriate care for memory‑care residents.
