I conducted unannounced investigations at Ivy Park at Simi Valley (facility number 565850299) in response to complaints received by the California Department of Social Services Community Care Licensing Division. The facility, located at 5300 E. Los Angeles Ave. in Simi Valley, operates as a type 740 group home with a capacity of 175 residents. Two distinct complaint control numbers were examined: one involving allegations of bed‑bug infestation (29‑AS‑20240423112621) and another encompassing multiple concerns about toileting, feeding, and medication management (29‑AS‑20241103120219).

The bed‑bug investigation began after a resident’s room had shown evidence of infestations for approximately three months. Housekeeping discovered bed‑bug signs on 03/04/2024, leading to a treatment by the pest‑control company Ecolab on 03/05/2024 and a follow‑up visit on 03/06/2024. Subsequent inspections on 04/22/2024 and 04/25/2024 recorded no pest activity, and a heat treatment was scheduled for 05/01/2024. Interviews with the resident, staff, and other residents revealed no ongoing infestations, and the facility’s record reviews indicated no pest activity during regular monthly visits. Because the evidence did not substantiate the claim that staff failed to prevent bed‑bug presence, the allegation was marked Unsubstantiated.

The second complaint set covered staff performance in meeting residents’ toileting and feeding needs, and medication management. Multiple visits (11/08/2023, 04/15/2024, 07/29/2024, and 09/24/2024) involved resident file reviews, medication checks, and staff interviews. For the toileting allegation, records showed routine status checks every two hours and staff assistance with dressing and dining, with no documented neglect. For feeding, documentation confirmed staff offered meals multiple times and monitored residents’ intake, with no evidence of inadequate feeding. The medication mismanagement claim was noted as Substantiated in the preliminary report, but the latest investigation concluded the allegation remained Unsubstantiated due to insufficient evidence. Overall, the investigations revealed a pattern of diligent documentation and routine oversight by Ivy Park, resulting in all current complaints being deemed Unsubstantiated.
