PARK VISALIA ASSISTED LIVING (Facility #547208809) was subject to a series of unannounced inspections by the California Department of Social Services between June 2021 and March 2022. The first visit, conducted on 14 June 2021, was a routine health‑and‑safety assessment that found no immediate concerns and required no corrective action. However, a subsequent case‑management inspection on 19 August 2021 revealed deficiencies in incident‑reporting procedures: multiple reports were filed late, and a resident’s medication had been obtained during an outing and returned to the facility. A plan of correction was issued and the deficiency was later dismissed by 25 October 2021.

In late August 2021 the facility’s infection‑control status was reviewed; a positive COVID‑19 case in the past seven days was noted, but the inspection was postponed and no tour was conducted. A September 24 visit merely amended the earlier report. By November 18 2021, a complaint investigation uncovered knives left in an unlocked kitchen drawer, constituting a safety hazard and a breach of California Code of Regulations, Title 22, Section 87705. The same inspector, in March 2022, documented an incident where a staff member yelped at a resident while caring for them, highlighting a personal‑rights concern.

Across the six visits, recurring themes emerged: lapses in timely incident reporting, inadequate medication and kitchen safety protocols, and staff conduct that threatened resident dignity. Although one reporting‑deficiency was dismissed, other cited issues—particularly the unlocked knives and staff‑resident interaction—remained unresolved. The pattern underscores a need for robust compliance monitoring, especially in areas of resident safety, personal rights, and incident documentation, to prevent future violations and protect vulnerable residents.
