Jefferson’s Ferry is Long Island’s first Life Plan Community, a not-for-profit Continuing Care Retirement Community at One Jefferson Ferry Drive in South Setauket, Suffolk County, New York, owned by Active Retirement Community Inc. Newsweek ranked it #1 in New York State and #28 nationally in its 2026 America’s Best CCRCs list, placing it in the top 1.5% of approximately 1,900 communities nationwide. The Vincent Bove Health Center, its named on-site health facility, carries a 5/5 CMS Overall Star Rating across all five subcategories, and U.S. News & World Report designated it a Best Nursing Home for 2026 with High Performing ratings in both Short-Term Rehabilitation and Long-Term Care simultaneously.

The community has been Medicare and Medicaid certified since November 6, 2001. Its defining feature is the Life Care contract: residents entering independent living receive guaranteed access to assisted living, memory support, and skilled nursing at predictable rates with little to no increase over their independent living monthly fee. The campus holds 248 independent living units, 60 private assisted living apartments, and 60 private skilled nursing rooms. Minimum admission age is 62. Those entering directly into assisted living from outside the community pay approximately $256 per day.

The New York State Department of Health, Office of Aging and Long Term Care is the inspecting body. Four inspections are on record, all with citations, against a state average of nine with five carrying citations. The most recent, a complaint investigation on June 13, 2025, identified 6 citations and 7 deficiencies against state averages of 29 and 22. The one deficiency related to freedom from abuse and neglect was corrected by July 31, 2025. Earlier inspections cited isolated life safety and resident rights issues, all corrected within reasonable timeframes. No enforcement actions or fines appear in the record. Thirty complaints have been filed at a rate of 50 per 100 occupied beds, marginally above the state average of 49.9; none were substantiated.

The Pinnacle Quality Insight Customer Experience Award for skilled nursing has been received six consecutive years. Fitch Ratings upgraded the community to BBB+ with a Stable Outlook in September 2025. Waitlists exist for one-bedroom-with-den and larger units, consistent with the high occupancy the Fitch upgrade reflects. Confirmed amenities include a fitness center, indoor pool, wood shop, movie theater, library, beauty and barber salon, restaurants, and gardens. Chef-prepared meals, housekeeping, medication management, and personal care are all included. Vision and dental care are available on-site.

Stony Brook University Hospital, ranked 10th in New York and High Performing in 19 procedures, is 2.2 miles away. John T. Mather Memorial Hospital in Port Jefferson is 3.0 miles distant.

Jefferson’s Ferry suits active adults 62 and older planning ahead for the full care continuum, particularly those for whom the financial predictability of a Life Care contract is a priority.
