Ten inspections between May 2019 and April 2025 identified a single deficiency, averaging 0.2 per year, which is 96% better than Oklahoma’s average of 4.9. The most recent April 23, 2025 complaint investigation documented one deficiency: the facility failed to submit an incident report to the state within one business day for one resident involving unwanted sexual advances between residents. Executive Director Fidelia Oglesby admitted to not reporting this allegation to the state agency initially.
