With 37 years of operation, the facility has established a stable market presence in a very walkable neighborhood (Walk Score 72).

Current occupancy reaches 69.7%, above Oregon’s state average; with a payer mix weighted heavily toward Medicaid residents (90.5% of current census), along with Medicare short-term admissions and smaller private-pay placements.

Since 2023, inspectors documented 73 deficiencies across eight health inspections and complaint investigations; an annual rate of 24.3 per year versus the state average of 6.7, placing performance 263% above the deficiency benchmark. Recurring themes span care planning and comprehensive assessments, resident rights and dignity, medication management accuracy, prevention of accidents and medication errors, and maintenance of safe and clean environments.

While no critical citations were issued and serious citations numbered just one, the cumulative pattern demonstrates systemic gaps. An $8,000 civil money penalty was assessed in April 2025 for regulatory noncompliance. No payment denials have been imposed.

Long-stay residents report depressive symptoms at 14.8%; that’s more than triple the state average of 4.6%, while falls resulting in major injury occur at 4.4%, nearly double the state rate of 2.4%. Pressure ulcer prevalence at 9.8% exceeds the state benchmark significantly. Antipsychotic medication use at 19.3% runs substantially above state norms.

Pneumococcal and influenza vaccination rates trail state standards considerably. Walking ability declines, however, occur in only 14.4% of long-stay residents; substantially better than the state average, and weight loss remains minimal at 0.7%.

Nursing hours average 5 hours 14 minutes per resident daily, matching state levels. Staff-to-resident ratios of 3.29:1 exceed state averages.

On-site amenities include a wellness clinic, exercise room, library, art center, greenhouse and raised gardens, and a dining program featuring chef-prepared meals with accommodation for dietary restrictions.

The facility accommodates long-term Medicaid residents, though its inspection trajectory and quality outcome patterns suggest careful review of current protocols before placement decisions.

The regulatory record documents serious compliance challenges from 2023 through early 2025 followed by recent improvement. Over three years, Maryville logged 63 deficiencies; 21 annually, 213 percent above Oregon’s 6.7-deficiency benchmark.

A January 31, 2023, complaint investigation and state licensure survey revealed ten deficiencies in comprehensive and quarterly assessment timeliness, resident records accuracy, quality assurance committee function, and administration compliance. An immediate jeopardy situation was identified and abated during this inspection. An April 7, 2023, complaint survey documented ten deficiencies spanning abuse prevention and investigation, diabetic foot care, nutrition and hydration maintenance, lab and hospice service delays, and antibiotic stewardship; some were corrected, others persisted.

An August 17, 2023, complaint survey cited nine deficiencies in abuse prevention, timely ADL assistance, change-of-condition notification, care plan implementation for transfers, staffing sufficiency, and infection control; mixed correction status. A December 18, 2023, complaint and state licensure survey found fourteen deficiencies covering resident rights violations, change notification failures, quality of care, accident hazards, elopement risk, bowel and bladder care, respiratory care, medication error rate at or above 5 percent, and drug storage; some corrected at revisit, others not.

A February 6, 2024, focused infection control survey identified one COVID-19 NHSN reporting deficiency uncorrected at revisit. A March 20, 2024, complaint survey cited four deficiencies in fall prevention, care plan implementation, and infection control with some remaining uncorrected.

A January 3, 2025, complaint survey found four deficiencies in accident hazard supervision and family notification; partial correction. An April 11, 2025, complaint survey revealed seven deficiencies in ADL care, infection prevention, employee reference and criminal record checks, and water management program implementation; some corrected at revisit, others not.

An October 30, 2025, complaint and re-licensure survey found zero deficiencies. The January 2023 immediate jeopardy finding and the pattern of deficiencies from 2023 through April 2025, particularly spanning abuse prevention and investigation, ADL care timeliness, accident hazard supervision, infection control, medication error rates at threshold, and assessment and documentation compliance, document fundamental care and systems vulnerabilities.

Responsiveness to the October 2025 revisit with zero deficiencies represents marked improvement, though sustainability requires confirmation during placement and tour.
