Laurel Parc at Bethany Village is a 153-bed luxury senior living community at 15850 NW Central Dr in Portland’s Bethany neighborhood, administrated by Executive Director James Stacy. The facility describes itself as prioritizing resident well-being and maintains a state-of-the-art wellness center with classes, rooftop terrace, movie theater, and Himalayan Salt Therapy Room. Three on-site restaurants serve chef-prepared meals with seasonal menus and a full-time pastry chef; special dining events include candlelit dinners and barbecues. Inspection record shows significant clinical and operational concerns. The most recent inspections by the Oregon Department of Human Services occurred in early 2023: a re-licensure survey on March 27, 2023 identified 13 deficiencies; an earlier state licensure inspection on January 17, 2023 found 5 deficiencies. Over the recorded period, Laurel Parc logged 18 total deficiencies across 2 inspections. While this is 42 percent fewer than Oregon’s average of 31 deficiencies per facility, per-inspection deficiency counts are markedly elevated: Laurel Parc averaged 9 deficiencies per inspection versus Oregon’s state average of 4.4; 105 percent worse. Substantive deficiency themes included failure to monitor changes in resident condition, incomplete RN assessments, medication errors, inadequate restraint assessments, insufficient staff training, and fire and life safety instruction gaps. Kitchen cleanliness, infection prevention, and licensing rule compliance were also cited. Some deficiencies were corrected on revisits; others persisted initially. No fines, immediate jeopardy findings, enforcement actions, or substantiated complaints were reported. The facility has not undergone inspection since March 2023, leaving a three-year gap in regulatory oversight.
Occupancy, pricing, and staffing composition are not disclosed.
Families should ask about corrective actions taken since March 2023 for medication management and clinical documentation, and fire and life safety protocols.

Arcadia Senior Living is a 74-bed assisted living community on SE Foster Rd in Portland, Oregon, nine years into operation and licensed by Oregon’s Department of Human Services, Aging and People with Disabilities. Administrator Kevin Call runs the facility, which the community describes as family-owned. The two-story building sits in a part of southeast Portland where a Walk Score of 34 means most trips out will need a car or a ride.
The apartment lineup spans three sizes: studios at 350 square feet, one-bedrooms between 569 and 750 square feet, and two-bedrooms at 904 square feet. That range is meaningful. A family moving a parent out of a large home can find something compact; a couple who want to stay together have room for that too. Respite care is available for short-term stays, which makes the community an option for families testing the fit before committing to a permanent placement or bridging a gap after a hospital discharge.
Oregon DHS inspections at this facility have focused on staffing and staff credentials, and care planning and documentation. These are standard regulatory focal points for assisted living in Oregon and give a general sense of where state oversight tends to look.
For older adults in southeast Portland who need hands-on daily support and prefer a smaller, independently run setting, Arcadia covers the basics at a scale where individual attention is at least structurally possible. The respite option adds flexibility for families still figuring out next steps.

Amenities include a pool, movie theater, fitness center, salon, library, and a calendar of activities including fitness programs, arts and crafts, and social gatherings. The facility reports itself as having received Best Independent Living and Best Memory Care awards from U.S. News and World Report.

Since 2022, Russellville Park has undergone 4 inspections (43% fewer than Oregon’s average of 7), with 21 total deficiencies (32% fewer than state average of 31). However, deficiencies per inspection average 5.3 compared to Oregon’s average of 4.4; 20 percent higher. Food sanitation and kitchen cleanliness issues recur across multiple inspection cycles. The most recent inspection on July 31, 2025 identified 1 deficiency related to kitchen practices and food sanitation rules. May 21, 2024 found 4 deficiencies including kitchen cleanliness, food sanitation, administration compliance, and inspection intervals. December 6, 2022 identified 12 deficiencies including failure to provide adequate activities, resident evaluations, service plans, health services, fire and life safety instruction, and staff training; November 29, 2022 found 4 deficiencies (kitchen cleanliness, food sanitation, inspection intervals, administration compliance). Some deficiencies were corrected on revisits; others persisted. Eleven licensing violations were recorded. No fines, immediate jeopardy findings, or enforcement actions were reported. Complaint investigations were not noted.
Families should ask about kitchen sanitation and corrections for recurring deficiencies, staff training (given the 2022 staff training deficiencies), and fire and life safety procedures.

Oregon Medicaid does not directly pay room-and-board for assisted living, but most states (including Oregon) offer Home and Community-Based Services (HCBS) waivers that can offset the cost of care services delivered inside a licensed community. Eligibility, waitlists, and covered services vary — check directly with the state Medicaid agency.
