Licensed nurses and certified nursing assistants provide on-site staffing. While this is 42 percent fewer than Oregon’s average of 31 deficiencies per facility, per-inspection deficiency counts are markedly elevated: Laurel Parc averaged 9 deficiencies per inspection versus Oregon’s state average of 4.4; 105 percent worse. Substantive deficiency themes included failure to monitor changes in resident condition, incomplete RN assessments, medication errors, inadequate restraint assessments, insufficient staff training, and fire and life safety instruction gaps. Kitchen cleanliness, infection prevention, and licensing rule compliance were also cited. Some deficiencies were corrected on revisits; others persisted initially. No fines, immediate jeopardy findings, enforcement actions, or substantiated complaints were reported. The facility has not undergone inspection since March 2023, leaving a three-year gap in regulatory oversight.

Occupancy, pricing, and staffing composition are not disclosed.

Families should ask about corrective actions taken since March 2023 for medication management and clinical documentation, and fire and life safety protocols.
