State inspectors conducted 10 visits within the first nine to ten months of operation; substantially above the state average of 8. During this period, the facility recorded two deficiencies. While this count trails the Illinois average of 7, both identified issues center on fall prevention and incident reporting, suggesting a pattern rather than isolated lapses.

The October 2025 substantiated complaint documented the facility’s failure to maintain timely, updated service plans for residents at high fall risk. Among the residents reviewed, one resident had 33 documented fall incidents; two others had 8 and 5 falls respectively. Despite these documented histories and fall-related injuries, service plans were not revised after falls occurred.

A month earlier, in September, inspectors substantiated a separate complaint: the facility failed to report a resident’s fall that resulted in a right femur fracture to the Illinois Department of Public Health, a regulatory requirement. These findings compound to indicate fall management and compliance reporting are operational challenges.

The facility’s January 2025 annual licensing survey found full compliance, and an earlier complaint investigation regarding COVID-19 showed no establishment failure.

Services include memory care with ADL assistance, medication management, secured units, activity programming, and support services such as transportation and housekeeping.
