Dogwood Creek Retirement Center is an 80-bed independent and assisted living community at 3230 East Shawnee Avenue, Muskogee, Oklahoma, Muskogee County, in downtown Muskogee, administered by Dani Woodland. The facility has received 2020’s #1 Assisted Living in Muskogee by the Muskogee Phoenix and 2018 and 2020 Best of Muskogee Awards.

Over 16 inspections spanning 2019 to October 2025, the facility accumulated 14 deficiencies. March 2023 renewal and complaint investigation found 7 deficiencies involving infection control, kitchen sanitation and food service training, resident assessments, activities staff qualifications, and CPR training gaps. July 2023 complaint investigation identified 2 deficiencies in kitchen maintenance, food storage protocols, and resident room conditions including mold and water damage. January 2021 COVID-focus survey found 2 deficiencies in infection control during meal service and bed bug infestation.

October 2025 renewal identified 2 deficiencies in quality assurance committee operations and expired medications in the medication refrigerator. March 2024 complaint investigation substantiated one deficiency involving inadequate supervision during showering resulting in a resident fall. Most other complaint investigations were unsubstantiated. No fines, immediate jeopardy findings, or license suspensions documented.

Improvement trend evident: 2020-2024 largely compliant with isolated deficiencies; recent 2025 findings more focused on administrative and medication management protocols.

Dogwood Creek provides 10 amenities: one/two-bedroom suites, three daily meals with individualized dietary accommodation, kitchenette in units, weekly maid service, 24-hour staffing, smoke detectors, emergency call buttons, full recreational/cultural/social programming, and wooded residential setting.

Occupancy is 78.8 percent (63 residents), in line with OK avg 60.6 percent.

Families should ask about medication management oversight and expired medication protocols, supervision procedures during bathing and showering, and quality assurance monitoring and committee operations.

Kyrie Assisted Living, LLC is a 43-bed memory care center at 801 Stadium Drive, Ada, Pontotoc County, Oklahoma, 2.9 miles from downtown. Licensed by the Oklahoma State Board of Health, the facility offers memory care services. Current occupancy is 32 residents from a 43-bed capacity (74 percent). Nursing hours averaged 28 percent above the Oklahoma state benchmark.

Private pay; Medicaid and Medicare not accepted. Prior operator was Brookdale Senior Living Communities, Inc.

The regulatory record reflects excellent compliance with minimal deficiencies over seven years. Over seven years, Kyrie logged four deficiencies, averaging 0.6 annually, 88 percent better than Oklahoma’s 4.9-deficiency benchmark. A May 9, 2019, license renewal found no deficiencies. An August 21, 2019, relicensure survey cited three deficiencies: failure to ensure laboratory tests were obtained as ordered by physician for two of three sampled residents, failure to ensure finger stick blood sugars were obtained as ordered by physician for one sampled resident, and failure to ensure blood glucose levels were obtained as ordered for a resident receiving home health services for blood glucose monitoring.

Substantial compliance was achieved by October 21, 2019, confirmed upon November 4, 2019, revisit. A January 17, 2020, license renewal found no deficiencies. A June 23, 2021, license renewal found no deficiencies. A February 14, 2022, license renewal found no deficiencies.

An April 13, 2023, complaint investigation addressed allegations of failure to notify legal surrogates of COVID-19 outbreak, inadequate staffing, and uncleanliness with offensive odors; all three allegations were unsubstantiated. One unrelated deficiency was cited: failure to ensure two of three dietary staff responsible for food preparation had attended an approved food service training program. Substantial compliance was confirmed by July 10, 2023. A December 15, 2023, initial licensing inspection found no deficiencies.

A July 10, 2024, license renewal found no deficiencies. A November 20, 2025, relicensure survey with complaint investigations addressed allegations of inadequate staffing, failure to involve legal guardians in care planning, inadequate supervision, failure to provide discharge notices and paperwork, failure to timely assess and intervene for resident condition changes, and failure to maintain a safe, clean environment free of pests; all complaints were unsubstantiated and no deficiencies were cited. Administrator Lisa Winkfein. Occupancy has declined from 35 residents in August 2019 to 32 in November 2025.

No fines or license suspensions are recorded.
