The January 2023 immediate jeopardy finding and the pattern of deficiencies from 2023 through April 2025, particularly spanning abuse prevention and investigation, ADL care timeliness, accident hazard supervision, infection control, medication error rates at threshold, and assessment and documentation compliance, document fundamental care and systems vulnerabilities.
Responsiveness to the October 2025 revisit with zero deficiencies represents marked improvement, though sustainability requires confirmation during placement and tour.
