Evan Terrace Post Acute is a 96-bed post-acute skilled nursing and memory care facility at 421 SE Evans Street, McMinnville, Yamhill County, Oregon. Operated by Evan Terrace Snf Healthcare, LLC, the community provides 24-hour skilled nursing with rehabilitation, memory care, therapy and assessment services six to seven days weekly, case management, discharge planning, and specialized programs including joint replacement and orthopedic rehabilitation, stroke rehabilitation, cardiac rehabilitation with CHF protocol, fall prevention, pulmonary care, post-operative care, pain management, infusion therapy, vascular wound management, renal disease services, tracheostomy care, cancer recovery, bariatric care, and palliative and hospice care.
The regulatory record documents persistent and severe care quality and operational compliance failures. Over three years, Evan Terrace logged 76 deficiencies; 25.3 annually, 278 percent above Oregon’s 6.7-deficiency benchmark.
A critical June 9, 2025, complaint investigation and licensure survey revealed twenty-three deficiencies spanning abuse reporting and investigation, medication management, infection control, staffing, care planning, treatment services, tube feeding, resident records, dementia care, and administrative rules; the majority were corrected by July 3, 2025, though some persisted uncorrected.
A November 26, 2024, complaint and licensure complaint survey cited twelve deficiencies in abuse investigation, care planning, medication administration, staffing, facility assessment, tube feeding, and drug regimen review; mixed correction status was noted. An April 19, 2024, complaint, licensure complaint, relicensure, recertification, and state licensure survey documented twelve deficiencies in assessment accuracy, care planning, medication administration and storage, staffing adequacy, quality of care, pressure ulcer treatment, and resident records; some were corrected, others not. A December 20, 2023, complaint, licensure complaint, and state licensure survey identified eight deficiencies in abuse prevention and reporting, alleged violation investigation, medication administration, and resident records with mixed correction outcomes.
A June 16, 2025, survey found six deficiencies in quality of care, pressure ulcer treatment, training requirements, and administrative rules, all corrected by July 22, 2025. A March 4, 2025, survey cited five deficiencies in quality of care and rehabilitation services with mixed correction status. The November 18, 2025, survey identified four deficiencies in tube feeding management and administrative rules, all corrected by December 2, 2025. Complaint investigations on October 17, 2025, July 31, 2025, and February 26, 2025, cited deficiencies uncorrected at visit.
The June 9, 2025, twenty-three deficiency cluster, particularly spanning abuse reporting and investigation, medication management, infection control, staffing, and care planning, reflects fundamental operational vulnerability.
While some facility responsiveness to revisit audits was documented, the persistence of uncorrected deficiencies across complaint investigations from December 2023 through October 2025 signals incomplete corrective implementation and systemic operational fragmentation. No fines or license actions are recorded; complaint investigations were unsubstantiated.
The facility provides post-acute skilled nursing, rehabilitation, memory care, specialty services, and palliative care with 24-hour staffing and therapy services. Nursing hours averaged 3 percent below the Oregon state benchmark. Private pay is required; Medicaid and Medicare are not accepted. Families should ask about corrective action documentation from the June 9, 2025, inspection, addressing abuse reporting and investigation, medication management, infection controls, staffing, and care planning, and require on-site verification of sustained implementation.

A March 27, 2023, change of ownership survey revealed eight deficiencies spanning change of condition monitoring, building exterior and interior maintenance, administration compliance, and behavioral health service plans; several were corrected by July 5, 2023, revisit.
A January 27, 2023, kitchen inspection found sanitation and repair issues mirroring the February 2024 findings, with partial correction by April 18, 2023, revisit.
Complaint investigations were unsubstantiated. No fines or license actions were recorded. The record shows food service and staffing documentation as recurrent problem areas, with some facility responsiveness to revisit findings yet without sustained resolution.
