A March 4, 2025, survey cited five deficiencies in quality of care and rehabilitation services with mixed correction status. The November 18, 2025, survey identified four deficiencies in tube feeding management and administrative rules, all corrected by December 2, 2025. Complaint investigations on October 17, 2025, July 31, 2025, and February 26, 2025, cited deficiencies uncorrected at visit.
