The community provides post-acute rehabilitation, short-term skilled nursing, memory care, assisted living, and home care services, with nursing care averaging 3 hours 47 minutes per resident daily (1 percent below Ohio average).
Inspection findings over the past six years warrant careful attention. Two falls with injury in December 2024 stand out as the most significant regulatory event. A resident with dementia and mobility impairments sustained rib fractures on September 27 after a therapy assistant left him unattended in the rehabilitation gym, and a broken wrist on October 5 after staff placed him at a table for a snack without supervision. Both incidents reflected failures in fall prevention protocols and ongoing staff supervision.
The surveyor found that the facility understood the resident required constant monitoring and contact assistance to prevent falls, yet staff repeatedly left him unattended. Prior deficiencies in August 2023 included unsecured medication left in an unlocked entryway (43 medication cards and pills in an open paper bag on a public table), kitchen sanitation failures with grease buildup and food debris on equipment and storage surfaces, and failure to convey a deceased resident’s funds within 30 days of death. A July 2024 complaint investigation substantiated misappropriation of a resident’s debit card and cash during a suspected staff theft, with the facility issuing a permanent suspension pending ATM video evidence. In December 2025, the facility failed to place a resident’s new hearing aids in her ears despite family requests and physician orders, leaving the resident attempting to insert them herself with shaky hands.
That same month, an x-ray technician entered a COVID-positive resident’s room wearing only a surgical mask instead of the required N95, violating droplet precaution protocols. Earlier deficiencies in December 2019 involved failure to continue winter activities and dine-in programs a resident specifically requested, and serving room-service trays at unappetizing temperatures.

Pneumococcal vaccination reached 100 percent.
Falls with major injury at 3.5 percent track with state average; pressure ulcers at 4.1 percent run slightly above state average at 3.9 percent.
Occupancy stands at 89.7 percent, 14 percent above state average, with predominantly short-stay Medicare rehabilitation residents (31 percent, 24-day average stay) and private-pay short-stay residents (65 percent, 4 to 5-month average). The facility generated $20.2 million in revenue with $1.5 million net profit. Staffing includes 26 registered nurses, 46 licensed practical nurses, and 148 certified nursing assistants (35 percent better staff-to-resident ratio than state average).
Families should ask about corrections following the September and October 2024 falls, supervision during therapy and meals, medication security, kitchen sanitation, and infection control compliance.
