Earlier inspections showed a pattern of issues with timely incident reporting, medication management, fire hazards, and staffing levels, with corrective actions implemented after each review. Prior deficiencies included fire safety concerns, medication labeling and storage problems, staffing shortages, and incomplete resident support plans. Complaint investigations were mostly unsubstantiated except for one substantiated case involving a delayed report of a resident’s acute fracture, which led to verbal education and a plan of correction.
