The California Department of Social Services has repeatedly inspected the Long Beach‐based residential care facility currently known as “Bay Towers at Bixby Knolls,” formerly “Laurel at Long Beach.” Administrator Don McDonald has been the consistent point of contact across all visits. The reports from October 2022, December 2022, and May 2024 consistently affirm that the facility meets the regulatory requirements set out in Title 22 of the California Code of Regulations.
During the October 2022 Component II telephone interview, the administrator confirmed a clear understanding of all key compliance areas—including staffing qualifications, medication management, abuse reporting, and the COVID‑19 mitigation plan. No deficiencies or complaints were documented at that time. The December 2022 pre‑licensing inspection found the facility in full compliance: resident rooms were well equipped, lighting and safety devices were operational, fire and emergency plans were in place, and food, medication, and hazardous‑material storage complied with state and federal standards. The evaluator noted no obstacles to egress and no safety hazards.
The May 2024 unannounced annual inspection reaffirmed the facility’s consistent adherence to regulatory standards. The evaluator verified adequate food supplies, safe medication storage, functional smoke and carbon‑monoxide detectors, and a recently conducted disaster drill. Records for staff and residents met all required documentation and clearance criteria. The building’s exterior and interior layout posed no safety concerns, and no bodies of water or other hazards were observed.
Across all three visits, the facility demonstrated a strong culture of compliance and safety. No complaints, citations, or deficiencies were reported, and the repeated evaluations indicate a stable environment that meets the Department’s expectations for adult residential care. The continuity of leadership, the thoroughness of safety protocols, and the lack of regulatory findings collectively suggest that the facility maintains high standards of resident care and operational integrity.
