This deficiency substantiates Complaint Number OH00132640 related to inadequate pain management and staffing issues.
Findings
The facility was found deficient in multiple areas including failure to provide dignified dining service, inadequate pain management following a fall, insufficient staffing on the Special Care Unit, medication errors exceeding acceptable rates, expired medications storage, improper food portioning and preparation, failure to accommodate resident food preferences, and unsanitary food delivery practices.
