Orchard View Post-acute and Rehabilitation Center is a 180-bed skilled nursing community in Kingsport, Tennessee, providing post-acute rehabilitation, long-term nursing care, and specialized medical support for older adults with complex healthcare needs. Operating since 1985, the community accepts Medicare, Medicaid, and private pay. However, weekend registered nurse staffing performs notably better than state benchmarks, and total weekend nursing coverage exceeds the Tennessee average. The most significant staffing concern is turnover, which remains substantially higher than statewide norms and may affect continuity of care and staff familiarity with residents’ needs.

Quality outcomes are mixed but include several encouraging indicators. Long-stay residents experience lower-than-average rates of falls with major injury, urinary tract infections, pressure ulcers, depressive symptoms, and antipsychotic medication use.
