Inspectors substantiated a complaint about unclean and odorous conditions in a resident’s room at that time, and the facility received a formal order to comply with state regulations. A follow-up visit in April 2024 confirmed that all previous deficiencies had been corrected and no new issues were found. This indicates improvement in compliance and resolution of prior concerns.
