Inspection Report Summary for Sage Glendale

SAGE GLENDALE Senior Living Facility (Facility #198603413) has undergone a series of licensing and compliance visits from December 2020 through July 2024. The initial application, completed in December 2020, was processed under a delayed‑egress license with the administrator and applicant demonstrating full understanding of Title 22 requirements. No deficiencies were reported, and the facility was instructed to submit the required LIC 809 documentation.

In March 2021 a pre‑licensing inspection was conducted virtually through FaceTime because of COVID‑19 restrictions. The five‑story complex, with separate memory‑care and assisted‑living wings, was found to be clean, well‑maintained, and compliant with fire, sanitation, and resident‑care standards. Fire extinguishers, smoke and carbon‑monoxide detectors, and emergency plans were all functional, and the facility received a fire‑safety clearance on January 28, 2021.

The first mandatory one‑year inspection in March 2022 focused on COVID‑19 infection‑control practices. While most protocols—mask use by staff, hand‑sanitizer stations, and isolation procedures—were in place, the inspector noted a significant water‑temperature deficiency in the first‑floor restrooms (74 °F to 113.7 °F), falling outside the 105–120 °F range required by CCR 87303(e)(2). A plan of correction was issued with a due date of March 25, 2022. The visit also documented adequate PPE, medication storage, and emergency preparedness, though residents were not observed wearing masks.

In April 2024 an unannounced case‑management incident visit reviewed a reported overdose of prescribed medication by Resident #1. The investigation revealed no actual ingestion, and the incident was deemed a reporting error. No health or safety issues were identified, and the facility remains pending reappraisal following the resident’s hospital discharge. A second unannounced incident visit was conducted on July 3, 2024; the narrative was not provided, but the visit was recorded as a routine follow‑up on an unspecified incident.
