The California Department of Social Services Community Care Licensing (CCLD) conducted four unannounced evaluations at Silverado Senior Living – Calabasas, a 740‑type facility with a 110‑resident capacity. The second evaluation, 7 June 2021, focused on a self‑reported incident involving two residents engaging in sexual activity in a resident room. LPAs Aja Richardson and Salia Walker interviewed the administrator, toured the facility, and reviewed the incident file. No immediate health or safety concerns were observed, the investigation was deemed incomplete, and no citations were issued. The third visit, 14 June 2021, was a required annual assessment emphasizing infection‑control practices during the COVID‑19 pandemic. LPA Ashley Smith toured the kitchen, bedrooms, restrooms, and common areas, confirmed proper signage, and verified symptom‑screening and PPE protocols. The inspection found no deficiencies.
The final evaluation, 23 August 2021, was a comprehensive case‑management investigation into previously substantiated allegations of neglect and supervisory failure that contributed to the death of Resident #1 (R1) on 1 July 2018. LPAs reviewed documentation, interviewed staff and medical personnel, and examined R1’s continuous glucose monitoring records. The investigation confirmed that the facility failed to monitor R1 after insulin administration, failed to seek timely medical care when R1 became unresponsive, and that staff were not competent or adequately trained to supervise R1’s care. As a result, the licensee received citations under Title 22 sections 87464(f)(1), 87465(g), 87411(a), and 87405(d)(1)(2), and a civil penalty of $500 was imposed.
Across the four visits, Silverado Senior Living demonstrated routine compliance with infection‑control and health‑safety standards, but the August investigation revealed significant gaps in resident monitoring and emergency response that led to serious regulatory sanctions. The pattern underscores the importance of ongoing staff training, vigilant supervision of residents with chronic conditions, and prompt medical intervention protocols to prevent future adverse outcomes.
