Licensing Program Analysts Renese Howell‑Small and Melody Brown documented that the two‑story building, which houses 79 resident bedrooms and matching bathrooms, meets all structural and safety standards. Heating and cooling systems, fire clearance, emergency supplies, and resident records were all in compliance, and the facility’s single entry point, resident activities, and staff provisions satisfied the California Code of Regulations. An unannounced complaint investigation on 21 November 2024 addressed allegations that staff were serving low‑quality food to residents. Evaluator Beena Singh interviewed all five residents present and reviewed food service records. All residents reported satisfaction with the food quality, and no deficiencies were found under Title 22, Division 6. The allegation was therefore determined to be unsubstantiated, and no corrective action was required.
A subsequent investigation on 31 December 2024 examined a more serious allegation that staff mismanaged resident medication. The complaint, filed on 3 October 2024, involved a resident who was discharged from the hospital on 28 September 2024 but did not receive the prescribed medication until 1 October 2024. In‑person interviews, a medication administration record audit, and review of the physician’s discharge notes confirmed that the facility failed to provide medication in accordance with CCR 87465(a)(2). The allegation was substantiated, and the report lists a Plan of Corrections requiring medication training for all staff and submission of proof to the Licensing Program Analyst by 2 January 2025. Failure to correct the cited deficiency may result in a civil penalty assessment. Overall, the series of reports demonstrates that while the facility largely complies with regulatory standards, it has experienced a significant lapse in medication management that requires immediate corrective action.
