Belmont Village Thousand Oaks received three unannounced complaint investigations during 2021‑2022, each concluding with an unsubstantiated determination. In February 2021, a resident’s bilateral hip fractures were reported following a seizure. The evaluator found no evidence of abuse; medical records, staff interviews, and a forensic review indicated that the fractures were consistent with the resident’s known osteoporosis, generalized weakness, and immobility. The incident was ruled a medical event, and no deficiencies were cited.

In June 2022, the facility was alleged to have failed to obtain timely medical attention for a resident who later died and to have delayed informing the resident’s family of a change in condition. Extensive document reviews, interviews with staff, the resident’s family, and paramedic logs revealed that the resident had a DNR order and had repeatedly communicated a desire not to be transported to a hospital. Because the resident’s death was deemed consistent with their advance directive and the family was aware of the situation, the agency found no evidence to support claims of neglect or reporting failure. No citations were issued.

The September 2022 investigation concerned an alleged illegal eviction and a resident’s elopement. The evaluator met with facility personnel and reviewed relevant documentation. The agency determined that the allegations could not be substantiated and closed the case without citations.

Across all investigations, a consistent pattern emerged: complaints were thoroughly reviewed, but the evidence failed to demonstrate any regulatory violations. No corrective actions or citations were imposed, and the facility maintained compliance with state licensing standards during the period covered.
