The December 2, 2024 complaint investigation identified 12 deficiencies including failure to support resident council meetings, inadequate injury investigations, and poor food temperature documentation. Earlier substantiated complaints included a 2023 elopement incident with inadequate reporting and a 2022 medication transcription error causing resident harm.
