During the reporting period from August 1, 2021 to July 31, 2025, Hollis Park Manor Nursing Home received a total of 30 citations. These citations were divided into 12 Standard Health Citations and 18 Life Safety Code Citations, for a combined total of 30 citations. The citations were identified during four on‑site inspections; an additional inspection was conducted during which no citations were issued.
