The California Department of Social Services’ Community Care Licensing Division conducted a series of unannounced case‑management visits to PACIFICA SENIOR LIVING MERCED (facility number 247206921). Each interaction was carried out by Licensing Program Analyst Lady Cabrera via telephone due to COVID‑19 precautions. The first two visits (January 28 and March 4, 2021) addressed an unusual incident/injury report filed on January 21, 2020. In the March visit, the agency documented that the licensee failed to meet California Code of Regulations, Title 22, § 87468.1(a)(3) concerning residents’ personal rights, and a deficiency was cited on the LIC 809‑D form. The administrator, Tyler Wilds, received an exit interview and the corresponding appeal rights documentation.

Throughout these encounters, the facility consistently provided requested documentation and was reminded of its appeal rights. The pattern indicates ongoing challenges in medication management and adherence to personal‑rights regulations, prompting repeated deficiencies and corrective action requirements.
