Silverado Thousand Oaks, LLC, a 740‑bed licensed facility in Thousand Oaks, California, was the subject of an unannounced complaint investigation conducted by Community Care Licensing Division evaluator Ashley Smith on February 16 2022. The investigation stemmed from a July 21 2021 complaint alleging that a resident (Resident #1) fell on July 16 2021, suffered multiple injuries—including rib fractures—and was not transported to the hospital until the following morning. Interviews, surveillance video, and medical records confirmed that staff were positioned approximately twenty feet behind the resident during the fall, a distance that precluded timely intervention. Nursing staff had assessed the resident as having only superficial wounds and, after the resident tore off steri‑strips the next morning, admitted that they could not determine the presence of internal injuries. Hospital records later documented recurrent falls, sinus bradycardia, periorbital and scalp hematomas, and both old and new rib fractures. The findings substantiated the allegations of inadequate supervision and delayed medical care, and the facility was cited for failure to correct these deficiencies.
In addition to the fall incident, the complaint included an allegation that Resident #1 was left in soiled clothing during the night of the fall. The investigation noted that an outside‑agency caregiver accompanied the resident from 9:00 p.m. to 6:00 a.m., after which another caregiver began work at approximately 7:55 a.m. Evidence suggested that the resident’s incontinent needs were not adequately met, resulting in the soiled clothing incident. While this claim was not fully substantiated, it highlighted broader issues of staffing coordination and resident hygiene management at the facility.
The final report required Silverado Thousand Oaks, LLC to submit a plan of action detailing staff training and fall‑prevention protocols by February 18 2022 and to submit a statement of understanding regarding emergency medical response by the same date. A civil penalty of $500 was assessed for the unaddressed deficiencies. The report underscored a pattern of insufficient supervision, delayed medical intervention, and lapses in basic resident care, indicating a need for systemic improvements in staffing, training, and emergency response procedures.
