- November 19, 2024 inspection cited 1 deficiency for minor cleaning, repair, equipment maintenance, hair restraints, and food storage issues under Food Sanitation Rules.
- March 2023 revisit corrected many of the 17 deficiencies found in November 2022 related to resident care, infection control, medication tracking, staff training, and fire and life safety.
