- November 13, 2025 re-licensure inspection found six deficiencies including failure to complete acuity-based staffing tool, inadequate staff training, unlocked poisons storage, and noncompliance with memory care rules.
- Multiple inspections from January 2023 through November 2025 repeatedly cited failure to fully implement acuity-based staffing tool and insufficient direct care staff training and competency.
- December 28, 2023 and December 8, 2022 licensure inspections found kitchen sanitation and food safety deficiencies, mostly corrected on revisits except for some unresolved items.
