The October 2023 renewal inspection occurred with minimal occupancy (4 residents, 4 staff) and found 10 deficiencies: admission agreements were signed by relatives instead of residents, and critical agreement content was omitted; service plans failed to reflect a resident’s hospice involvement; tuberculosis screening was never completed for at least one resident; staff training was incomplete; medications were administered daily despite orders specifying as needed only; menus were not posted, meal census records did not exist, and kitchen food was opened, unsealed, and undated.

The October 2024 complaint investigation revealed the most serious violation. A resident developed urinary tract infection symptoms, with visible signs including behavioral changes and redness. No contact was made to the attending physician until one month later. State investigators determined that a one-month delay in medical attention for a potential UTI constituted noncompliance with regulations.

Annabelle Cosibitt is administrator. Current occupancy is 4 residents in 48 beds, 8.3 percent of capacity. The license. No fines or enforcement actions are recorded.

The facility opened with massive physical plant violations, then demonstrated wholesale failure to implement basic admission, medication, and resident protection procedures. The October 2024 medical attention delay shows that fundamental clinical decision-making is broken. One-month delays in responding to visible urinary tract infection symptoms represent not carelessness but structural failure in how the facility prioritizes resident health.
