Laurel Oaks of South Haven is a 64-bed community at 05499 73rd Street, South Haven, Van Buren County, MI, operating as a family-owned Home for the Aged within the city center. The facility serves residents in assisted living and memory care, with specialized programming for residents experiencing Alzheimer’s disease and dementia. Respite care is available for temporary stays. The neighborhood is very walkable, scoring 72 on accessibility metrics and ranking 63rd of 702 Michigan communities in this category.
The community features multiple gathering and activity rooms, a movie theater, beauty salon, full-service commercial kitchen, grand dining room, private dining room, spa room, and exercise room. These amenities support a range of social and recreational activities alongside residential care.
The facility opened in January 2023 and received an original licensing inspection confirming substantial regulatory compliance with zero deficiencies.
Its first renewal inspection followed in June 2023, approximately five months after opening. That renewal identified eight deficiencies spanning administrative, documentation, and operational areas. Inspectors documented: failure to formally notify the state of an administrator change; missing tuberculosis screening records for one resident; unsigned service plans for two residents; absence of meal census documentation for the preceding three months; gaps in ventilation system monitoring; absent hot water temperature monitoring; missing dishwasher sanitization records; and unlabeled and undated food items in refrigerators.
Census at the June 2023 renewal was 18 residents occupying a 64-bed facility. No complaint investigations or enforcement actions appear in available records. The deficiencies reflect documentation and environmental monitoring issues rather than direct care concerns, but indicate areas requiring corrective attention in a newly opened facility.
Laurel Oaks of South Haven suits residents seeking assisted living or memory care in a highly walkable setting with diverse recreational amenities.
New Hope White Lake Senior Living Community is a 117-bed assisted living and memory care facility at 450 S Williams Lake Road in White Lake, Oakland County, Michigan. Operated by New Hope White Lake, LLC, with Khurram Rumi Shahzad as Authorized Representative and Owner, the facility offers assisted living and memory care services under an all-inclusive pricing model. Administrator Shannon Snapp oversees operations. Respite care and life enrichment programming are available, along with assisted living lite services.
The facility is car-dependent, ranked 642nd of 702 statewide by walk score of 6. Current occupancy stands at 47 percent, with 55 residents of 117 capacity as of May 2025. The facility opened in January 2023.
Original licensing in January 2023 documented substantial compliance with zero deficiencies. Within six months, July 2023 renewal identified three deficiencies. The facility was offering independent living services without department approval, a program statement issue rather than operational failure.
It failed to maintain a meal census record as required. A sink in the assisted living salon had very low water pressure.
October 2023 complaint investigation substantiated two deficiencies following allegations of medication impropriety and HIPAA breaches. Medical records were accessible in an unlocked copy room. Medications were not always administered per physician’s orders. Documentation gaps included failure to initial medication administration records and unclear duplicate medication orders.
Allegations about inadequate resident care, inadequate memory care staffing, and building maintenance issues were not substantiated based on investigation findings. May 2025 complaint investigation substantiated one deficiency for documentation errors in medication administration records and controlled substance logs. The investigation found no evidence that residents failed to receive prescribed narcotic medications, but several documentation errors existed. An allegation about resident skin breakdown from inadequate personal care was not substantiated.
The pattern across four inspections over 2.3 years reveals systemic documentation deficiencies and administrative lapses rather than evidence of withheld care. The medication documentation failures are material for compliance purposes. The HIPAA breach from unlocked medical records is serious and warrants questions about privacy protocols and corrective action. The facility’s compliance posture appears as a newer operation still calibrating administrative systems, particularly around medication recording and privacy controls, though care delivery allegations have not been substantiated by investigators.
Families should verify medication administration protocols, documentation safeguards, and HIPAA compliance improvements following the October 2023 and May 2025 substantiated deficiencies before placement.

Serene Gardens of Hartland is a 79-bed assisted living community at 2799 Bella Vita Drive in Hartland, Livingston County, Michigan, operated by 2799 Bella Vita LLC. Originally licensed as Bella Vita of Hartland in August 2020, the facility underwent a name change in July 2023. Services include assisted living, memory care, and respite care. Walk score is 8, indicating car-dependent location.
Original licensing in August 2020 documented substantial compliance with zero deficiencies. Administrative review in February 2023 confirmed zero deficiencies and continued substantial compliance. This clean record extended through the July 2023 name change. But beginning January 2024, the picture changed materially.
A January 17, 2024, renewal inspection with 18 residents and 9 staff present identified 6 deficiencies: no supervisor of resident care was designated for each shift; medication administration records were incomplete; posted therapeutic diet menus did not reflect actual modifications served, meal census documentation failed to record food quantities served, and kitchen condiments lacked expiration dating or were outdated; and soiled linen room ventilation was inadequate.
Fourteen days later, on January 31, 2024, an investigation substantiated admission protocol failure: resident A was admitted without physician orders for prescribed medication, resulting in missed medication doses. Investigators found that Resident A did fall but staff responded appropriately, and no violation was established for the fall response itself.
By August 13, 2024, pattern became concrete. A complaint investigation documented that Resident A experienced 13 falls. The facility’s incident reporting system captured only 5 of those falls in written reports. Other complaint allegations regarding privacy breach and maintenance issues were unsubstantiated following investigation.
Administrator Megan Rheingans and Authorized Representative Daniela Popaj represent current leadership. Eight deficiencies over four years places the facility at 62 percent better than Michigan’s average of 5.2 per year.
Families should recognize that recent deficiencies, while the facility remains below state average overall, represent a meaningful shift from prior compliance.

Frequently Asked Questions about Assisted Living in Michigan
What's the difference between assisted living and memory care in Michigan?
Assisted living in Michigan supports residents with daily activities (bathing, dressing, medication management) while preserving independence. Memory care is a specialized form of assisted living for residents living with Alzheimer's or dementia, and adds 24/7 secured environments, dementia-trained staff, and structured routines designed to reduce confusion and wandering.
What's the difference between assisted living and a nursing home in Michigan?
Assisted living in Michigan is a residential model focused on housing, hospitality, and help with daily activities. Nursing homes (skilled nursing facilities) provide 24/7 medical care from licensed nurses for residents with significant health needs, and are regulated more strictly under both state and federal CMS rules.
Does Michigan Medicaid cover assisted living?
Michigan Medicaid does not directly pay room-and-board for assisted living, but most states (including Michigan) offer Home and Community-Based Services (HCBS) waivers that can offset the cost of care services delivered inside a licensed community. Eligibility, waitlists, and covered services vary — check directly with the state Medicaid agency.
What is assisted living?
Assisted living combines private or semi-private apartments with help on activities of daily living (meals, bathing, dressing, medication management) and a social calendar — for residents who need some support but do not need 24/7 medical care.
How many assisted living facilities are listed on this page?
This page features 438 assisted living facilities in Michigan. Use the filters and comparison tools above to compare ratings, amenities, and pricing.
How do I choose the right assisted living facility in Michigan?
Start by matching the level of care offered to the resident's current and anticipated needs, then compare licensing status, staff-to-resident ratios, recent inspection results, and pricing. Tour at least two or three communities in Michigan, talk to current residents and families, and confirm what is included in the base rate versus billed as add-on services.
What should I look for when visiting assisted living facilities in Michigan?
Pay attention to staff interactions with residents, cleanliness and odor, food quality at meal times, the activity calendar, and how questions about pricing and care plans are answered. Ask to see the most recent state inspection report, the move-out / level-of-care-change policy, and a sample monthly bill that lists every fee.
