Programs include life enrichment activities, religious services, and transportation. Private pay; Medicaid and Medicare not accepted.

The regulatory record reflects strong compliance over six years with one critical deficiency in 2025. Over six years, Saint Ann logged one deficiency, averaging 0.2 annually, 96 percent better than Oklahoma’s 4.9-deficiency benchmark. Relicensure surveys on June 27, 2019, and August 2, 2019, found no deficiencies. An August 18, 2020, COVID-19 special focus infection control survey found no deficiencies.

Relicensure surveys on December 31, 2020, and August 1, 2021, found no deficiencies. A May 25, 2022, complaint investigation addressing allegations of failure to ensure licensed personnel administered medications found the complaint unsubstantiated with no deficiencies. A May 23, 2023, relicensure survey found no deficiencies. A May 28, 2025, relicensure and complaint investigation addressing allegations of inadequate supervision to prevent elopements found no deficiencies.

However, a June 4, 2025, complaint investigation substantiated one critical deficiency: the facility failed to ensure medication was administered per physician orders for one resident, resulting in two missed doses. The medication error likely contributed to the resident’s disorientation and elopement from the unlocked facility. A June 20, 2025, license renewal authorized continued operation with a maximum capacity of 40 beds. The facility’s occupancy has declined from 46 residents in June 2019 to 31 residents in June 2025.

No fines or license suspensions are recorded.

The facility provides assisted living, independent living, memory care, respite care, and rehabilitation services with 24-hour staffing. The June 4, 2025, substantiated medication administration error resulting in two missed doses and contributing to resident elopement represents a serious patient safety incident despite the facility’s otherwise excellent deficiency record. The occurrence of the June 4 deficiency just one week after the May 28 relicensure found no deficiencies signals a breakdown in medication administration oversight.
