The Life Safety inspection on August 8, 2025, conducted by the Office of the State Fire Marshal, produced 8 deficiencies, all related to fire safety maintenance: storage near sprinkler heads, exposed wiring, power tap misuse across multiple rooms, ceiling and door penetrations, and an obstructed fire extinguisher. All 8 were corrected during the inspection. A similar fire inspection on July 8, 2025, had flagged related issues. The February 14, 2025, annual inspection found 2 administrative deficiencies: missing dementia training documentation for two staff members and an incomplete resident service agreement regarding electronic monitoring access. Both were resolved the same day. No fines, enforcement actions, or substantiated complaints reported. Meanwhile, staffing is a standout. Rockwood South Hill logs 5 hours and 46 minutes of total nursing care per resident per day, ranking 4th out of 89 Washington skilled nursing facilities and running 32 percent above the state average. Registered nurse hours reach 1 hour and 36 minutes per resident per day, 81 percent above the Washington average of 53 minutes, which translates to stronger clinical oversight throughout the day. The facility holds a CMS Health Inspection rating above the state average, though its overall 3-star CMS rating reflects a Quality Measures score trailing the state benchmark.
Amenities include a fitness center, multiple dining venues, outdoor spaces, and salon and spa services.
Rockwood South Hill suits residents seeking post-acute short-term rehabilitation or long-term skilled nursing care in a larger non-profit campus.
