A December 1, 2023, complaint investigation addressing abuse allegations found no deficiencies.

A January 3, 2025, complaint investigation addressing abuse and property misappropriation allegations found no deficiencies. The facility’s policy required two staff members for all transfers, but one certified nursing assistant transferred the resident alone, violating protocol and directly contributing to a fall and subsequent resident death. The staff member was terminated. A revisit on January 12, 2026, confirmed all deficiencies were corrected by December 31, 2025. Administrator Michelle Netters.
