A September 11, 2024, complaint investigation addressing allegations of physical, verbal, or psychosocial abuse was not substantiated and found no deficiencies. An October 14, 2025, relicensure survey found no deficiencies.

The facility’s occupancy has grown steadily from 37 residents in February 2020 to 65 residents in October 2025. No fines or license suspensions are recorded. The August 2024 substantiated abuse incident was addressed through immediate termination and staff re-education; the October 2025 relicensure survey with zero deficiencies indicates sustained compliance recovery and operational focus on resident safety.
