Most state complaint investigations resulted in unsubstantiated findings; the facility’s history includes one substantiated complaint (2021) involving inadequate discharge notice wording, now corrected, and one recent deficiency (November 2025) related to hand hygiene during food plating in a secondary kitchen, corrected within six weeks.

Falls with major injury occur in only 1.7% of residents; 61% better than state performance, while antipsychotic medication use stands at 4.4%, far below the state average of 15.5%. Functional decline and depressive symptoms, however, present challenges: functional decline scores run 54% worse than state averages, and depressive symptoms affect 6.1% of long-stay residents versus the state average of 3.3%.

Montereau suits private-pay and Medicare residents seeking established continuing care in a financially stable, well-staffed community, though functional outcomes and mental health support warrant careful individual assessment.
