The December 2019 relicensure survey substantiated 2 deficiencies including a contractual refund failure and medication administration errors. The April 2019, January 2023, March 2022, and June 2021 renewal licenses found zero deficiencies.

The February 2021 and November 2022 complaint investigations were unsubstantiated. No fines or enforcement actions appear in the available record.
