Medication management has been a recurring focus in deficiencies. Sixteen inspections from June 2019 through March 2025 yield 2.2 deficiencies per year, 55 percent better than Oklahoma average of 4.9. The March 2025 licensure inspection found 3 substantive deficiencies: incomplete lunch meals for 6 of 8 residents not meeting registered dietitian approval, failure to report abuse allegations to state and nurse aide registry for 1 resident.

The July 2022 complaint survey substantiated 1 medication deficiency with three residents experiencing 15 missed doses due to inaccurate medication records. Earlier inspections from 2020 through 2021 and February 2020 found zero or minimal deficiencies. The December 2019 relicensure survey substantiated 2 deficiencies including a contractual refund failure and medication administration errors. The April 2019, January 2023, March 2022, and June 2021 renewal licenses found zero deficiencies.
The February 2021 and November 2022 complaint investigations were unsubstantiated. No fines or enforcement actions appear in the available record.
Occupancy has declined significantly from 45 residents in 2020 to 27 in March 2025, suggesting market challenges or capacity constraints. Tulsa is the second largest city in Oklahoma; Saint Francis Hospital and Hillcrest Medical Center are major nearby resources.
