Heritage Point is more than just a memory care facility; it’s a memory care community. These include physical, social, emotional, cognitive, and spiritual nurturing, all aimed at enabling your loved one to lead a fulfilling life within a safe and secure environment.
Brickyard Healthcare – Lincoln Hills Care Center is an 86-bed facility in Tell City, Indiana. Owned by Ggnsc Tell City LLC and operated under Hendricks County Hospital with administrator Julie Pennington, the home runs at a 73 percent occupancy rate and accepts Medicare, Medicaid, and private pay. The average length of stay spans 223 days, indicating a resident population that blends short-term rehabilitation patients with long-term clinical placements.
A daily nursing care average of 3 hours and 23 minutes per resident supports specialized programs in stroke recovery, dialysis support, diabetes management, pulmonary treatment, and pain control. The 24-hour staffing team works with visiting physicians and on-site pharmacy, dental, podiatry, and vision services to minimize outside appointments. The campus features private suites, a beauty shop, religious services, and organized activities, while the surrounding neighborhood scores a 55 for walkability, making it moderately walkable for visitors running errands.
State inspectors have reviewed emergency preparedness plans, generator operations, sprinkler systems, and medication handling, with the team utilizing standard corrective paths to maintain compliance.
Families can contact the main desk to check on room availability, ask about the scheduling for on-site medical specialists, or set up a building tour. The front office can also explain insurance options and help you handle the initial registration forms.
Waters of Wabash Skilled Nursing Facility West is a newly opened skilled nursing home in Wabash, Indiana. At 44 beds, the community is operated by Johnson Memorial Hospital and administered by Kiri Burks. The facility located on Alber Street provides short-term rehabilitation and ongoing skilled nursing care for residents who require therapy services and nursing support. Medicare, Medicaid, and private pay are accepted, giving families several options when arranging care.
Residents stay an average of 103 days, including those recovering from hospitalization, surgery, injury, and other health conditions that require post-acute care. Daily nurse staffing averages 2 hours and 57 minutes per resident. Registered nurses, nurse aides, and licensed practical nurses provide hands-on care throughout the day and night; tere’s around-the-clock staffing to support the facility’s focus on rehabilitation and skilled nursing services.
Specialized programs include Living Well and In Motion, both designed to support residents during the recovery process. The facility also provides in-house diagnostics, comprehensive wound care, and respiratory services. Through these services, many residents can receive needed treatments and therapies without leaving the community. Respite care is available for families who need short-term placement while caring for a loved one at home.
State inspections previously identified concerns related to fire safety and emergency preparedness. The facility addressed those issues, and later inspections confirmed compliance in both areas. Follow-up reviews reflect continued attention to resident safety and operational safeguards. The location has a Walk Score of 26, so the setting is car-dependent. However, some nearby services are within walking distance.
As a newly opened facility, Waters of Wabash Skilled Nursing Facility West has another skilled nursing option for families in the area with a focus on an operational model centered on rehabilitation outcomes and nursing-level care.
Operated by Wyndmoor Of Evansville Llc, The Wyndmoor of Evansville is a 110-bed facility at 6521 Greendale Drive in Evansville, Indiana. Administered by Jessie Moore, it’s in its first year of operation with 90 residents occupying beds, yielding an 82 percent occupancy rate. The setting scores 11 on the Walk Score, so most errands demand a vehicle in this car-dependent neighborhood.
Inspection records demonstrate recurring themes surrounding medication administration practices, medication storage, food storage conditions, mental health assessments, and hand hygiene protocols. Some earlier complaint investigations had no substantiated deficiencies.
During a tour, people may want to ask the home about pricing, staffing hours, payment acceptance, care type confirmation, services, programs, dining options, activities, and amenities.
Miller’s Merry Manor is a 127-bed nonprofit nursing home in Logansport, Indiana, offering skilled nursing, memory care, respite care, and short-term rehabilitation services. The facility is operated by Anna Foster and accepts Medicare, Medicaid, and private pay. The campus is 0.0 miles from the city center at 200 26th Street and serves residents across Cass County.
CMS rates the facility at 5 stars overall, with above-average scores across all four rating categories — health inspection, staffing, quality measures, and the composite overall. Nurse staffing averages 4 hours and 20 minutes per resident per day, roughly 23% above the Indiana average of 3 hours and 32 minutes, with RN staffing specifically 25% above state benchmarks.
The inspection record complicates the facility’s ratings. Across roughly a dozen surveys since 2022, the facility has averaged 10.3 deficiencies per year — 145% above Indiana’s average of 4.2 — with the heaviest concentration in the May 2025 annual survey, which cited 20 life safety code violations involving fire door operability, sprinkler maintenance, emergency lighting records, and fire drill timing.
Three complaints were substantiated over the period: a 2022 case involving missed showers and unsafe mechanical-lift transfers that resulted in fractures for two residents; a 2024 case involving a resident moved from the memory care unit without the POA’s consent; and a 2024 infection-control lapse during a COVID outbreak. No fines, immediate jeopardy findings, or license actions appear in the record.
Rehabilitation services include the Real. Life. Rehab. and Clinical Pathways tracks. Amenities include restaurant-style dining, religious services, a beauty/barber shop, and a wheelchair-accessible van for community outings.
Miller’s Merry Manor is best suited for residents seeking post-acute skilled nursing or memory care in the Logansport area, particularly families who value staffing ratios and CMS ratings alongside a compliance record that reflects ongoing life-safety maintenance challenges. However, families should review the facility’s higher deficiency totral ad on-going life-safety maintenance issues before considering placement.
A community that is filled with a professional and dedicated care staff who strive to make a difference in their residents’ lives, West Bend Nursing and Rehabilitation offers a continuum of nursing home services– respite care, hospice care, skilled nursing, and rehabilitation; as well as focusing on assisted living and memory care service. The elegant nursing home is a resident-centered community where residents can thrive and reach their fullest potential to be a new version of themselves.
West Bend strives to ensure excellent hospital-like service in a comfortable, homey environment. Services at the nursing home are interdisciplinary, focusing on the specific healing process for each resident: outpatient therapy, long-term care, and short-term care; activities and programs focus on functional communication, cognitive skills, and physical movement. As convenience is the heart of their service, medical services and physicians are available in the community.
Operated by Rush Memorial Hospital and administered by Justin Lai, Flatrock River Lodge is a newly established skilled nursing and rehabilitation community in Rushville, Indiana. The facility provides nursing services and therapeutic support following surgery, illness, and hospitalization. Medicare, Medicaid, and private pay are accepted, giving families several options for arranging care. With 114 beds and 28 current residents, the community is in its growth phase and offers a quieter country setting with nearby walking paths and local ties.
Daily nurse staffing averages 4 hours and 25 minutes per resident. Registered nurses, nurse aides, and licensed practical nurses provide hands-on care throughout the day. This level of nursing support reflects the facility’s focus on rehabilitation and recovery. Residents stay an average of 111 days, which aligns with many post-acute recovery plans and supports individuals working toward discharge home or transition to a lower level of care.
The community combines rural surroundings with reasonable accessibility. Rushville has a Walk Score of 67, so some errands can be completed on foot. It’s located about an hour south of Indianapolis, where the regional health care resources are, while it preserves a neighborhood atmosphere.
Meals are overseen by a chef and dietitian who develop menus and accommodate a variety of dining preferences. Snacks are available three times each day between meals. Residents can enjoy outdoor spaces such as the gazebo, patio, and walking path if the weather is good. Additional amenities include a beauty salon, barber shop, private and suite-style rooms, dedicated family visiting spaces, shopping services, cable TV, and free Wi-Fi.
State surveys have identified areas requiring continued attention involving emergency preparedness and Life Safety Code compliance. The areas of focus were generator testing records, fire drill documentation, and equipment maintenance practices. Families visiting the community should ask staff how these systems are monitored and maintained to keep residents safe.
Brickyard Healthcare – Petersburg Care Center is a 63-bed skilled nursing facility in Petersburg, Indiana, offering long-term care, short-term rehabilitation, respite care, and outpatient therapy services, with 24-hour staffing and a clinical mix that includes dialysis, stroke, and pulmonary care, as well as cognitive training. The facility is operated by Brickyard Petersburg LLC under the governance of Hendricks County Hospital.
The facility holds a 4-star overall CMS rating, with all three sub-components — health inspection, staffing, and quality measures — scoring above the Indiana average. Across inspection visits since 2023, the facility has recorded 16 total deficiencies compared with an Indiana average of 36, roughly 56% below the statewide norm and averaging one deficiency per inspection visit. No critical or serious citations appear in the record, and no fines or enforcement actions have been issued.
Two substantiated complaint investigations — one involving wound documentation in late 2023 and one involving PICC line care that same year — resulted in citations but no penalties; subsequent revisits confirmed correction. The most recent inspection, a complaint investigation in November 2024, found zero deficiencies.
The facility received the 2018 AHCA/NCAL Bronze Quality Award in recognition of its performance improvement. Falls with major injury for long-stay residents average at 1.2%, compared to an Indiana average of 3.9%, and pressure ulcer rates for high-risk residents are 38% below the state average — both meaningful signals for higher-acuity long-term placement.
The facility is oriented toward long-term Medicaid residents, who make up 86% of the current census, with an average length of stay of 250 days. Its clinical service depth — dialysis, pulmonary care, and stroke rehabilitation available on-site — suits residents in Pike County with complex, ongoing medical needs who require more than basic custodial care.
The Villages at Historic Silvercrest is a 108-bed nursing home in New Albany, Indiana. Administered by Stephanie Miller and operated by the Board of Trustees of the Flavius J. Witham Memorial under owner Trilogy Healthcare of Floyd, the facility accepts Medicare, Medicaid, and private pay. For visitors, the local area has an exceptional walkability score of 81 out of 100, which means the surrounding neighborhood is highly pedestrian-friendly, with local shops and services reachable on foot.
A 71 percent occupancy level defines the daily community here, with individual stays averaging about 50 days. This timeline shows a functional focus on short-term post-acute care and transitional rehabilitation over permanent placement. Around-the-clock staffing manages this clinical load, delivering an average of 3 hours and 32 minutes of direct nursing care per resident each day, including 48 minutes from registered nurses and 2 hours and 21 minutes from nurse aides.
Official health department surveys highlight past regulatory citations focused on kitchen safety, fire procedures, and resident care records. Subsequent visits show that the facility has addressed these items with documented corrective actions, and the building maintains an overall citations-per-inspection rate of zero.
Interested individuals researching comprehensive senior layouts or transitional physical therapy plans can use these state regulatory filings to assess the property’s operational background. The public documents outline a pedestrian-friendly neighborhood campus that provides an indoor pool, fitness center, resident garden, and chapel, balanced against completed fixes in past care documentation and safety logging.
