Pineview Cottage

3498 Harbor-Petoskey Rd, Harbor Springs, MI 49740
Overview of Pineview Cottage

Pineview Cottage is a 40-bed assisted living and memory care community at 3498 Harbor-Petoskey Road in Harbor Springs, Emmet County, Michigan, operated by Pineview Cottage, LLC. Dean Bonesteel is Administrator. Walk score is 65, moderately walkable. BBB rating is A+.

The facility opened in 2018 with substantial compliance. The February 2023 renewal found zero deficiencies. This record collapsed in July 2023. Within 15 days, two separate Adult Protective Services complaint investigations identified 7 deficiencies spanning resident care, legal documentation, and discharge practices.

July 5, 2023, investigation found a discharge letter that omitted the resident’s right to file a complaint and improperly specified hospital as a discharge location, which is not an approved discharge setting. The investigation cleared allegations about incorrect discharge notice, disrespectful treatment, insufficient staffing, and medication non-administration.

July 20, 2023, investigation documented restraint violations and systemic documentation failures: resident B was restrained in a wheelchair with a seatbelt without a physician order specifying reasoning or timeframe; resident C was restrained with gloves or mitts on hands without physician order; resident B’s admission agreement was invalid because it was signed by a relative without activated durable power of attorney rather than by the resident.

The facility failed to develop an appropriate service plan for Resident B due to the same lack of activated durable power of attorney. No tuberculosis screening was on record for Resident B within 12 months before admission. The facility refused to accept Residents B and C back after hospital discharge, constituting improper discharge practice.

March 2024 renewal found continued problems. Resident A’s tuberculosis screening was completed after admission, not before. Employee A lacked evidence of tuberculosis screening within 10 days of hire.

The July 2023 deficiencies involved use of restraints without clinical authorization, acceptance of legally invalid admission documents, and refusal of appropriate hospital discharge placements. These are structural failures in how the facility manages resident rights and care decisions. The facility corrected neither the restraint practice nor the tuberculosis screening practice by March 2024. Occupancy increased from 45 percent to 58 percent during this period, suggesting the facility continued admitting residents despite unresolved deficiencies in restraint protocols and admission documentation.

Families should verify restraint authorization protocols, admission agreement procedures, and discharge acceptance practices with the facility directly before placement, given July 2023 deficiencies.

Overview of Pomeroy Living Northville Assisted & Memory Care

Pomeroy Living Northville Assisted & Memory Care is a 109-bed community at 40033 West Eight Mile in Northville, Wayne County, Michigan, operating under Beacon Square Northville. The facility provides assisted living, memory care, outpatient rehabilitation, short-term rehabilitation and recovery, hospice and palliative care, and respite care. Administrator Sandra Salvati and Authorized Representative Manda Ayoub lead operations. Walk score is 83, very walkable and ranked 24th statewide.

The facility opened in 2016 with substantial compliance. The inspection record from 2016 was clean.

However, September 2023 identified four deficiencies with 21 residents on-site. Resident A’s tuberculosis screening chest x-ray was completed after move-in rather than prior to admission as required; resident B missed scheduled doses of Tramadol on August 18, August 30, and August 31 with no documented reason for non-administration; resident C did not receive prescribed Ketoconazole shampoo applications from September 2 through September 6 as ordered every other day. The menu posted in the dining room listed meals for a prior week instead of the current week.

The September 2024 complaint investigation reveals a more substantive gap. Resident A experienced four falls during his stay due to inadequate supervision. The facility used a tilt back wheelchair seat as a restraint method, which the investigation determined to be inappropriate. A resident in the facility’s care fell repeatedly while staff supervision was insufficient to prevent injury.

The pattern across both inspection periods centers on administrative and direct-care compliance. Medication administration documentation gaps, screening timing gaps, and prescribed care delivery gaps appeared in 2023. A substantiated supervision failure appeared in 2024. The facility has matched the Michigan average for deficiency rate over ten years, which means its performance is unremarkable in a statistical sense. Yet the specific content of those deficiencies involves core care functions. For residents requiring close oversight or those with fall risk, the supervision failure documented in September 2024 is material.

Families should ask about the progression from clean compliance, to recent deficiencies in medication administration and substantiated supervision failure leading to resident falls.

What government services provide money for senior housing?

Several government programs offer financial assistance for senior housing in various countries, focusing on the United States for a broad overview:

  • Medicaid: Medicaid is a state and federally funded program that can cover the costs of nursing home care for those who meet eligibility criteria, including income and asset limits. Some states also offer Medicaid waivers that help pay for home and community-based services to prevent or delay nursing home placement.
  • Medicare: Medicare, primarily a health insurance program for people aged 65 and over, does not cover long-term housing costs. However, it can cover short-term stays in a skilled nursing facility under specific conditions following a hospital stay.
  • Section 202 Supportive Housing for the Elderly Program: This program provides housing for low-income seniors. It offers rental assistance and access to supportive services, such as cleaning, cooking, and transportation.
  • Low-Income Housing Tax Credit (LIHTC) Properties: While not a direct subsidy, LIHTC encourages developers to create affordable housing. Seniors with low incomes can find reduced-rent apartments through this program.
  • State and Local Programs: Many states, counties, and cities offer their own programs to assist seniors with housing costs. These can include property tax relief programs, rental assistance programs, and programs that offer affordable senior housing options.
  • Veterans Affairs (VA): The VA offers several programs for veterans, including the Aid and Attendance benefit, which provides monthly payments to veterans who require the aid of another person, or are housebound, to help cover the cost of care in homes, nursing homes, and assisted living facilities.
  • Social Security: While Social Security primarily provides retirement income, for many seniors, these benefits are a crucial part of their budget, including housing costs.
How do you pick the homes you recommend?

Before recommending homes, we conduct a thorough evaluation on crucial factors that define a senior home such as the quality of care they provide, the reputation of the organization, and a comprehensive review of community testimonials as well. Every detail is assessed to assure that seniors are offered not just senior care options, but trustworthy homes where they can experience the care that they deserve.

Do you help families that need Medicaid?

Yes. Assisting families who need Medicaid is important to us as it plays a crucial role in offering financial support to seniors confronting economic challenges. We would like to keep essential healthcare services accessible to all community members, irrespective of one’s financial circumstances.

We extend our services at no cost, ensuring that families and seniors can benefit from tailored assistance in their search for care and home options.
