Complaint surveys on September 23, 2025, March 6, 2025, August 21, 2024, and March 12, 2024, noted citations uncorrected at visit. Two January 2024 focused surveys cited COVID-19 NHSN reporting failures.

The January 2024 immediate jeopardy findings, the April 2025 twenty-one deficiency cluster, and the November 2025 chemotherapy medication order failure resulting in patient harm, constitute a pattern of fundamental care and systems failure.

While some facility responsiveness to revisit audits was documented, the persistence of uncorrected deficiencies across complaint investigations from January 2024 through November 2025 indicates incomplete corrective implementation and sustained operational fragmentation.
