Over six years, Providence Place logged five deficiencies, averaging 0.8 annually, 84 percent better than Oklahoma’s 4.9-deficiency benchmark. A May 9, 2019, license renewal found no deficiencies. A November 15, 2019, relicensure survey cited one deficiency: failure to ensure water temperatures did not exceed 115 degrees Fahrenheit in residents’ rooms with observed temperatures ranging from 115.1 to 128.3 degrees Fahrenheit in sampled rooms, representing potential harm to residents.

A December 1, 2020, COVID-19 focused survey cited one deficiency: failure to ensure residents were screened daily for signs and symptoms of COVID-19 for three of three sampled residents. The screening logs showed inconsistent documentation and the policy did not specify screening methods or frequency. This deficiency represented potential for more than minimal harm with no actual harm identified. An April 12, 2023, relicensure survey cited three deficiencies: failure to ensure resident assessments were coordinated and signed by a registered nurse or resident’s personal physician for two of eight sampled residents, failure to ensure resident assessments were signed by the resident or resident representative for two of eight sampled residents, and failure to develop care plans to reflect resident assessments for two of eight sampled residents.

A March 1, 2025, license renewal found no deficiencies, certifying continued operation with a maximum capacity of 52 beds. Administrator Ashley Miller. The facility’s occupancy has remained between 24 and 29 residents in recent inspections. No fines or license suspensions are recorded.

An October 5, 2023, complaint investigation substantiated one deficiency: failure to ensure food was stored, prepared, and served in a sanitary manner with an unclean ice machine containing black slimy substance on the deflector plate. Substantial compliance was confirmed upon revisit on January 4, 2024 with corrective action completed by October 31, 2023.

A November 8, 2023, initial licensing certification found no deficiencies. A May 7, 2024, license renewal authorized continued operation with a maximum capacity of 43 beds. A March 11, 2025, relicensure survey found no deficiencies. A November 5, 2025, complaint investigation substantiated one serious deficiency: the facility failed to provide adequate nursing supervision to ensure safe transfer needs were met for one resident, resulting in a fall and hospitalization.

The facility’s policy required two staff members for all transfers, but one certified nursing assistant transferred the resident alone, violating protocol and directly contributing to a fall and subsequent resident death. The staff member was terminated. A revisit on January 12, 2026, confirmed all deficiencies were corrected by December 31, 2025. Administrator Michelle Netters.

The facility’s occupancy has remained between 34 and 36 residents in recent inspections. No fines or license suspensions are recorded.
