A June 6, 2023, relicensure survey cited five deficiencies: failure to ensure food was stored, prepared, and served in a sanitary manner with uncovered trash cans and lack of beard guards for staff with facial hair, failure to ensure potentially hazardous leftovers were disposed of within 24 hours, failure to ensure resident assessments were coordinated and signed by a registered nurse or physician for two of eight sampled residents, failure to ensure comprehensive assessments included personal interviews for two of eight sampled residents, and failure to ensure direct care staff were trained in first aid and cardiopulmonary resuscitation for four of five recently hired staff members. A December 1, 2023, complaint investigation addressing abuse allegations found no deficiencies.

An October 10, 2024, complaint investigation addressing physical abuse allegations found no deficiencies. A January 3, 2025, complaint investigation addressing abuse and property misappropriation allegations found no deficiencies. The facility’s occupancy has remained between 47 and 73 residents in recent inspections. No fines or license suspensions are recorded.

The facility provides assisted living, long-term care, memory care, Parkinson’s care, respite care, and rehabilitation services. Licensed capacity is 88 beds with current occupancy of 73 residents (83 percent). Nursing hours 25 percent above Oklahoma benchmark. Administrator Angela Green.
