Chapters Living of Tulsa is a 46-bed memory care facility at 7807 S Mingo Road, Tulsa, Tulsa County, Oklahoma, 9.6 miles from downtown, offering specialized memory care with 24-hour nursing care, living areas with fireplaces, library, covered patio, dining room, chef-prepared meals, laundry service, beauty salon and barber shop, wheelchair-accessible showers. Current occupancy is 27 of 46 beds (59 percent) as of March 2025, below Oklahoma average of 60.6 percent, with occupancy declining from 45 residents in February 2020 to 27 in March 2025. The facility does not accept Medicaid or Medicare. Nursing care averages 25 percent above Oklahoma state benchmark.

Medication management has been a recurring focus in deficiencies. Sixteen inspections from June 2019 through March 2025 yield 2.2 deficiencies per year, 55 percent better than Oklahoma average of 4.9. Recent findings show operational compliance challenges, primarily in food service, medication administration, and incident reporting. The March 2025 licensure inspection found 3 substantive deficiencies: incomplete lunch meals for 6 of 8 residents not meeting registered dietitian approval, failure to report abuse allegations to state and nurse aide registry for 1 resident.
The facility submitted an acceptable plan of correction achieving substantial compliance by May 2, 2025. Earlier significant deficiencies appeared in 2023 and 2022. The June 2023 relicensure survey identified 3 deficiencies spanning dish machine sanitation, staff training gaps in CPR and first aid, and medication administration errors including crushing medications contraindicated for crushing and missing blood pressure monitoring. The March 2023 complaint investigation substantiated 2 deficiencies regarding inadequate heat and electrical outlets in resident rooms and failure to report fire and electrical outages.
The July 2022 complaint survey substantiated 1 medication deficiency with three residents experiencing 15 missed doses due to inaccurate medication records.
