In November 2024, an unannounced survey identified an infection-control violation with critical severity; meaning the noncompliance posed immediate risk of serious harm to residents. That citation was corrected. Two months earlier, a complaint investigation documented serious deficiencies in pharmacy documentation and quality-of-care practices; both were resolved. Resident-rights violations comprise the largest category (9 of 39 citations), followed by quality-of-life concerns (8 citations).

The facility incurred $38,000 in federal penalties between November 2024 and January 2025.
