Most recent inspection on November 20, 2025, a routine survey, found zero deficiencies. However, an inspection three months prior on August 28, 2025 identified five deficiencies: residents in the memory care unit did not receive cognitive activity programs as described in the residency agreement; kitchen floors were not clean and ice machine maintenance was not completed; staffing was insufficient to meet resident care and evacuation needs with call light response times exceeding 15 minutes in 55 instances; incident reports were not submitted timely; and medications were not available for one resident during a medication pass.

Prior to this, April 10, 2025 license renewal found zero deficiencies. An August 15, 2024 relicensure survey found two deficiencies covering staff training deficits; plan of correction accepted with correction confirmed September 30, 2024. A November 9, 2023 complaint investigation regarding medication administration found two deficiencies covering failure to administer medications according to physician orders and inaccurate medication records; plan of correction accepted with revisit confirmation February 9, 2024.

An earlier complaint investigation on January 14, 2020 substantiated allegations of improper medication administration with one deficiency cited for medication administration and record documentation.
